Employee Complaint Form

  • Please fill in the employee complaint form below if you want to file a complaint against one of your co-workers. It is company policy to investigate all complaints and take appropriate action. The form will be sent to a member of the MUST Board. Prior to filing out this form you should follow the normal chain of command with MUST. If you have an issue with one of your fellow employees then you should first report the issue to your direct boss. If your complaint is directed toward your boss you should first try and address the issue with him/her. If that does not resolve the issue, you should direct the issue toward his/her boss. If that does not resolve the issue then you can use this form. If the issue is of a sensitive nature and you do not feel that it is appropriate and/or safe to report the issue to your boss or his/her boss then feel free to use this form.
